Abstract   We studied the therapeutic effect of 5-fluorouracil in combination with low dose leucovorin in patients with recurrent or metastatic carcinoma of the colan or rectum. 40 patients with clearly measurable lesions were entered into this study between Sep. 1989 to May 1991 and two were excluded from the analysis because one patient did not meet the eligibility criteria and the other was lost to follow-up. They were treated with leucovorin, 20 mg/m/day iv bolus, followed by 5-fluorouracil 370 mg/m/day iv bolus for 5 consecutive days. This regimen was re- peated every 4 weeks and 5-fluorouracil doses were escalated to 420 mg/m,day if the previous dose was tolerable. Male to female ratio was 25 to 13 with median age of S4 years(32-81 years). 22 patients had previous history of chemotherapy and 16 were previously untreated. There was no comPlete response and the partial response rate was 26.4%(10 of 3B patients). The median response duration was 5 months and the median time to progression was 3 months. Hematologic toxicities were mild and the rates of grade 3 or greater nonhematologic toxicity were 7.8%(3 of 38 patients). There was no treatment-related fatality. Our results indicate that the combination of -fluorouracil and leucovorin has modest activity for patients with metastatic or recurrent carcinoma of the rectum and colan.
